HME and SelfCare is proudly partnering with Vancouver Coastal, Fraser Health and Vancouver Island Health Authority to provide home medical equipment to patients in need through the MEPP program in the areas where the above health authorities operate.
This joint partnership enables HME to help provide short-term and long term rental medical equipment that is prescribed by clinicians to their clients and patients in need, for a duration of time dictated by the prescribing clinician.
Together with SelfCare, there are 7 locations with over 100 employees ready to serve you in the lower mainland and Vancouver Island.

In order for clients and patients leaving the hospital or receiving health care services at home to receive the loan equipment, forms “MEPP Referral Form” and “MEPP Specifications List” must be filled out by the clinician.
Clinicians can get the MEPP program forms directly from their health unit/health authority.
The clinician is responsible for filling out the necessary information on the referral and equipment forms, including the rental time, before providing the forms to the client to be submitted to HME (along with any other additional instructions). The client is then responsible for faxing the form to the appropriate HME office that services their area.
After submitting the forms to us, HME will schedule a time with the family for delivery. Clients can also come into their local showroom to pick up items, depending on their location.
It is the clients responsibility to return the equipment to the vendor by the date on the referral form. Equipment must be returned to the same vendor they were loaned from.

It is the responsibility of the client, not the clinician, to return the equipment by the date indicated on their referral form. If there is a need to extend the rental period, the prescribing clinician must complete the “MEPP Extension Form“.
