ALS Equipment Loan Process

Prescribing therapists must request loans using the ALS Equipment Portal. If you are a new therapist and need access, email for information. If you are an ALS Patient, you must get your therapist to access the program. All loans must be handled by a therapist (prescription). HME as the subcontractor of ALS will fill orders once ALS sends the forms to HME. HME cannot process any requests without this. Each loan request must have a completed form or portal form done.

ALS Equipment Returns

For ALS equipment returns, please complete the attached forms and send them to Please note only certain equipment is returned to the pool (due to high costs to return the equipment, a lot of equipment is recycled or disposed of by the client after use).
